Photo and filming consent form

Thank you for agreeing to be part of our photography / filming.

By signing this form, you are giving your consent for Cottsway to use your photograph/film footage for publicity purposes. This may include, for example, release to the media, use within literature, publications and general public release to help illustrate our services, for example on our website and/or social media channels. The examples given are not exhaustive.

We are required to gain written consent via this form for every individual photographed/filmed. Under-18s must have parental/guardian consent – if you are completing this form for a child please include their name/s in the section below.

This information will only be used to confirm you have given us your consent, and to allow us to easily identify which photos / films you feature in should you withdraw your consent in the future.
Photographs/footage will be kept on our records for up to 4 years and may be used for publicity purposes more than once during this time.
After this time, we will delete your photographs/film from our records, however we cannot guarantee that images or footage will not continue to appear in the public domain (for example on printed literature or social media). By signing this consent form, you are agreeing that you understand this.
We will not sell these photographs / film with other organisations or agencies, however photographs may be shared with partner organisations where relevant, and these may be shared wider, over which we have no control.
