Case study, photo and filming consent form
Thank you for agreeing to be part of a case study, photos or filming.
By signing this form, you are giving your consent for Cottsway to use the information and / or your photograph or film footage for external communications purposes. This may include, for example, use in literature and publications, release to the media, and public use to illustrate our services and the impact of customer feedback, such as on our website and social media channels.. The examples given are not exhaustive.
We are required to gain written consent via this form for every individual photographed/filmed or featured in a case study. Under-18s must have parental / guardian consent – if you are completing this form for a child please include their name/s in the section below.

Please be advised:
- Case study information, photographs and film will be kept on our records for up to 4 years and may be used for communications purposes more than once during this time.
- After this time, we will delete your case study information, photographs or footage from our records. However, we cannot guarantee that information, images or footage will not continue to appear in material already in the public domain (for example on printed literature or social media). By signing this consent form, you are agreeing that you understand this.
- We will not sell case studies, photographs/ film to other organisations or agencies, however information or photos/ footage may be shared with partner organisations where relevant, and these may be shared wider, over which we have no control.
